Do I need two copies?
For consular NIE, Exteriores Edinburgh asks for the original and one copy. Police appointment pages may ask for the official form as part of the documentation.

Does EX-15 replace 790-012?
No. EX-15 is the request form; 790-012 is the fee form or receipt when the procedure requires payment.
